Health care professionals who wish to be removed from the OptumHealth Allies Web site should contact OptumHealth Allies Provider Relations at (877) 426-2559 or by selecting the Contact Us link.

E-mail Referrals
On designated areas of the Web site, you can request to have OptumHealth Allies information and OptumHealth Allies network provider information e-mailed to an address and an addressee you have independently provided. OptumHealth Allies will only use the furnished information for the singular request identified.

Security
OptumHealth Allies has security measures in place to protect the loss, misuse and alteration of the information under our control. The data you provide to us is stored in password-protected areas of OptumHealth Allies' computers, and is available only to a limited number of OptumHealth Allies employees. Health care services related to treatment performed on a child should only be sent to us by that child's parent or legal guardian, and OptumHealth Allies will not be able to act on behalf of any child unless we have received authorization from that child's parent or legal guardian. Changes to Your Information
Our Web site gives you the option to change, review and modify contact information and dependent information previously provided when you establish your account. You can review or modify information by logging into the "My Account" section of the Web site using the unique username and password established at registration, or by contacting Customer Care at (877) 426-2559 . Contact information and dependent information may only be changed once the member's identity has been confirmed by correctly answering certain identifying questions. As part of this process, a member may be asked to verify: (1) the member's OptumHealth Allies membership number; and (2) member demographic information.

Upon reviewing, changing, or modifying member contact information, changes are immediate and can be confirmed by the member on the "My Account" section of the Web site.

Notification of Changes to Web Site
This Statement may be modified from time to time. If OptumHealth Allies makes changes to this Statement, we will post a prominent notice on our Web site notifying users of the change. In addition, the revised statement will also be posted, and any such changes will be effective immediately upon posting.
